H1N1 FLU CLINICS
Mesa Public Schools is sponsoring FREE H1N1 Flu Clinics for those who are eligible:
* All children 6 months and older--including adults up to 25 years of age
* Pregnant women
* Persons who have a child under the age of 6 months living in their home
* MPS school staff with school ID
Consent forms will need to be completed--you can download and print out consent forms and vaccination information sheets and also find more in-depth information about the H1N1 flu virus at the District's website.

HEALTH OFFICE INFORMATION
Immunizations: Arizona state law requires that students be current on immunizations. The Health Office will send out notices to parents/guardians if their student's immunizations are not up-to-date. Immunization requirements for 2009/10 can be found on the District's web page .
The following organizations offer free immunization clinics:

Medications: Medications must be kept in the Health Office, in their original container labeled for that student. A consent form to keep medications in the Health Office and dispensed as ordered must be completed. Provisions can be made for students to carry their own emergency medications, but a consent form (for an inhaler or emergency med) must also be completed and kept on file.

Screenings: The Health Office does vision screenings (for all new students to MPS and special ed students), hearing screenings (for all 9th graders, new students to MPS and special ed students), and scoliosis screenings (for all 7th graders). If your child does not fit into one of these categories but you would like him/her screened for vision, hearing or scoliosis, please call and request a screening.
Chronic Health Conditions: If your student has a serious chronic health condition, such as anaphylaxis(severe allergy to a substance where an epipen is needed at school for a life-threatening allergic reaction), asthma, diabetes or seizures, please click on the condition, print out and complete the District health-care plan and bring it to the Health Office. We will put the care plan into place and make sure all of the student's teachers are aware of his/her condition.
