Enrolling Your Child
To register a child in MPS for the first time, take a certified copy of the student's birth certificate, current immunization record and proof of residence to the school office. Parents will be asked to fill out a registration form and an emergency medical referral card.
Immunizations
Arizona law requires documentary proof of immunity against certain
childhood diseases for students entering Arizona schools for the first
time. With some exceptions, students subject to this requirement who
lack documentary proof may enroll but not attend school until proof is
provided. If your child falls into this category, more detailed
information is available at the school.

times. Immunizations are
also available from family physicians and pediatricians.
Proof of residence
To show the child's home address, parents may provide one of the following: rent receipt, utility bill, motor vehicle registration, housing contract, lease agreement, state income tax return, bank account, paycheck, employer letter on letterhead.
Kindergarten
Full-day kindergarten is open to children who are 5 years old before Sept. 1. The schools also offer a half-day experience for parents who want this option. If a school does not have enough students enrolled in half-day kindergarten, a child may attend the full-day program for half the school day.

For children who will have their 5th birthday between Sept. 1 and Dec. 31, the district offers a variety of options.
